Start with responsibilities

An aftercare plan should explain who will do what after treatment. The CDC emphasizes continuity of care and obtaining records before returning home. Start by asking your treating team to identify the clinician or service responsible for scheduled follow-up, and the contact route for concerns outside those appointments.

Put these details in a short document that you can access without searching old messages. Include names, phone numbers, opening hours and the location of planned reviews. Check whether a contact is a clinical service or a booking desk, so you know what kind of help it can provide.

Describe the handover

If follow-up at home is part of the proposal, verify the arrangement directly with the named provider. Do not assume that a local professional has accepted responsibility because their city appears in a brochure. Ask what records they need and how an appointment will be arranged.

Create a handover checklist for the treating clinic to complete. It can include the procedure summary, relevant records, prescribed instructions and the date or conditions for review. Ask the team to explain unfamiliar terms while you are still able to speak with them directly.

Make the practical plan specific

Use separate sections for routine reviews, questions and urgent concerns. The treating clinician should tell you which symptoms require prompt attention and what action to take. Copy their instructions accurately instead of replacing them with a generic internet list.

Ask how extra appointments, tests or treatment would be charged and what any guarantee actually covers. A promise to discuss a concern remotely is different from an arrangement to examine or treat you. Record those distinctions in your budget and contact list.

  • Who reviews my progress, and where?
  • What written instructions will I receive?
  • Who can respond when the clinic is closed?
  • Which follow-up costs are included?
  • What records should I provide to a clinician at home?

Keep the plan available after the trip

Before leaving, check that you have the final version of the instructions and that the contact details work. Keep a secure copy available offline. If you choose to involve a companion, share only the information they need to help you follow the agreed arrangements.

Aftercare is not a task to mark complete at the airport. Keep upcoming reviews visible in your calendar and bring new questions to the appropriate clinician. This article organizes the handover; the treating team must provide your individual care instructions and advice about seeking urgent help.
